The North Fork Ambulance Association is offering the Vial of Life Program to all North Fork Valley residents. The Vial of Life Program is designed to speak for you when you are unable to speak for yourself in an emergency situation at your residence. The information contained within the vial, can provide our EMTs with essential details that will aid in providing appropriate medical treatment. The best part - its FREE!

Pick up your FREE Vial of Life Kit at one of the following locations:

  • Hays Drug - Paonia
  • Paonia Town Hall
  • City Market Pharmacy - Hotchkiss
  • Hotchkiss Town Hall
  • Crawford Town Hall
  • North Fork Medical Clinic - Paonia and Hotchkiss

What do I have to do?

It’s easy to do. Just complete the Medical Information Form, place the form in the Vial and place the Vial in your refrigerator. The kit comes with an ambulance magnet for the front of your refrigerator. Simply place the magnet on the front of your fridge, and our EMTs will look for it upon entering your residence. A list of your current medications may be requested from your pharmacy and included in the Vial.

Simply follow the steps below.

  1. Pick up your vial of life with all the necessary paperwork contained inside at one of the locations listed above.
  2. Complete the Medical Information Form
  3. Place the form in the Vial
  4. Place the Vial in your refrigerator on the top shelf of the door
  5. Place the NFAA Ambulance magnet on your refrigerator.
  6. Update your information regularly.
  7. Call 911 in the event of any emergency so that we can help you!
